What You'll Do

As an SSVF Case Coordinator, you will provide compassionate, individualized case management to veterans experiencing or at risk of homelessness. Working within the Housing First model, you'll partner with veterans to develop housing stability plans, connect them with community resources, and support them on their journey toward independence.

Responsibilities include:

  • Conduct eligibility screenings and enroll veterans into the SSVF program.
  • Develop and implement individualized Housing Stability Plans.
  • Meet regularly with participants to monitor progress and provide ongoing support.
  • Assist veterans in locating and maintaining safe, affordable housing.
  • Connect participants with employment, healthcare, budgeting assistance, and other community resources.
  • Determine and coordinate financial assistance to support housing stability.
  • Maintain accurate client documentation and data in the Homeless Management Information System (HMIS).
  • Collaborate with VA staff, landlords, community organizations, and referral partners.
  • Conduct outreach throughout the communities we serve.

What We're Looking For

Required Qualifications

  • Associate degree in Human Services or a related field or two years of experience in human services, case management, or a related profession.
  • Strong communication and organizational skills.
  • Ability to work independently while managing multiple priorities.
  • Proficiency with Microsoft Word, Excel, Outlook, and database systems.
  • Valid driver's license, clean driving record, and reliable insured vehicle.
  • Ability to pass required background checks.
